‘Creating a Solid Foundation'
Capital & Coast DHB is committed to improving the health of Pacific people and the Pacific Health Unit (PHU) was a step towards that goal.
The purpose of the PHU was to ‘Create a Solid Foundation' for Pacific who utilise our services.
The aim of the PHU service is to reduce the amount of time Pacific people spend in hospital and reduce the rate of re-admissions.
The Pacific Health Unit has team of staff comprising of:
- Team Leader
- Registered nurse
- Community Health worker
- Trainee Breastfeeding Lactation Consultant
- Pacific Midwife
- Administrator
- Pacific RNs from Primary care
The service has a unique model of care - ‘Home to Home' utilising the family approach. The model incorporates the cultural appropriateness, advocacy, support, collaboration, coordination, consistency, cohesiveness and integration with services and providers internally and externally whilst the patient and their family remain at the core.
Communication is key to all aspects of the Model but the patient and their family is the focus.
Our commitment for the future is to sustain a quality service and reduce inequalities and disparities for Pacific peopl.
Model of Service Delivery
The aim of the model is to provide seamless care and intersectoral intervention (as required) from the time patient steps through the ‘front door' and leaves the hospital to return home.
